Important Information for New Patients

Whether you are here for a routine check-up or have specific health goals in mind, we promise to provide you with the kind, high-quality care you deserve.
We will need the following information from your Primary Care or current physician in order to establish care.
- Demographic/face sheet
- Most recent office note from the referring physician ( we only need 1 )
- All test and lab results that pertain to the reason for the appointment. This includes Ultrasound, Biopsy, CT and MRI results if applicable.
( Diabetic patients - A1C is a must w/ a metabolic panel )
